Who we serve:

• Adults who are homebound due to illness, disability, or mobility limitations.

• Residents of assisted living or senior housing.

Before your first visit:

• Have your ID and insurance card ready.

• Prepare a list of your current medications.

• Share recent hospital or clinic documents if available.

During the visit:

• Our provider will review your history, medications, and current health concerns.

• Exams and on-site tests may be performed.

• A care plan will be created and shared with family or caregivers.

After the visit:

• Prescriptions, orders, and referrals will be sent as needed.

• Follow-up visits will be scheduled to manage ongoing health needs.
